Join us in making a positive impact on our residents' lives!Job Summary: SLP shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.Responsibilities/Requirements:Screen all residents in the facility on a quarterly basis, to identify those who have experienced declines in their functional abilities and determine if Speech Therapy intervention is indicated.Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual.Reassess patients and caseload every 30 days to determine need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated.Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment.Educate staff, family, and care givers as indicated.Identify needs for program development and implement programs according to company policies and procedures.Identify equipment needs of Speech Therapy Dept. and order equipment according to company and facility policy.Complete all required documentation according to guidelines specified in Documentation Manual.Coordinate in-services for facility staff as needed is determined, to ensure safe and effective carryover of Maintenance/Restorative Programs and overall call of the residents.Attend all facility required meetings.Promote profession within facility, including coordinating special events during National Speech Therapy Month.Attend Continuing Education courses specific to current position.Maintain professionalism and always follow all national/state associations regulations.Supervise student affiliates as directed.Education and Licensure:Must have a masters degree in Speech-Language Pathology from an accredited program.ASHA (CCC) CertificationActive state license required.Clinical Experience:Prior experience working as a Speech Therapist.Experience in geriatric care, addressing functional limitations, mobility challenges, and rehabilitation needs.Knowledge and Skills:Excellent organizational and time management skills.Excellent written and verbal communication skills.Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines.Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines.Communication and Interpersonal Skills:Excellent verbal and written communication skills to effectively interact with patients, their families, and interdisciplinary team members.Empathy and patience to work with individuals who may have physical challenges or limitations.A collaborative approach to working with other healthcare professionals and effectively communicating treatment plans and progress.Compliance and Documentation:Competence in maintaining accurate and timely documentation of evaluations, treatment plans, progress notes, and discharge summaries.Annual TB Gold Blood Test or questionnaire required.Continuing Education:Willingness to engage in ongoing professional development and continuing education activities to stay updated with advancements in the field.Physical Requirements:Ability to stand, sit, and move around for extended periods to evaluate and treat patients.Physical strength and dexterity to facilitate therapeutic activities, assist with transfers, and use equipment.Capacity to lift and position patients as needed during therapy sessions.Potential exposure to infectious diseases and human waste products.Prolonged periods of sitting and working on an electronic device.Functional Fundamentals:Commit to understanding and fully engaging in our Functional Fundamentals Program.Integrate our Functional Fundamentals into your role daily.Engage in our Functional Fundamental of the Week by discussing and incorporating into your week ahead.Qualifications:Must have a masters degree in Speech-Language Pathology from an accredited program.ASHA (CCC) CertificationActive state license required.#HP1
