Position Summary:
Coordinate and provide ongoing patient support, oversight and
education related to perinatal access. Serve as primary patient
contact for Special Delivery patients. Facilitate coordination of
genetic counseling appointments and flow of information between
internal and referring providers, clerical staff, and contracted
vendor. Collaborate with the MFM staff, Ultrasound staff, Outreach
staff, and Genetic Counseling staff. Serve as a clinical resource
and liaison to patients, family members, staff, and providers on
access and coordination of care. Provide clinical leadership and
quality oversight to clinical staff. Serves as a role model to
promote positive work environment and quality patient care. Ensures
adherence to Hospital and Departmental Policies and Procedures.
Ensure adherence to Hospitals and departmental policies and
procedures. Patient care assignment may include neonate, pediatric,
adolescent, adult and geriatric age groups.
Detailed responsibilities:
* ENVIRONMENT - Maintain a safe, comfortable, and therapeutic
environment for patients/families in accordance with Hospitals
standards
* ADMINISTRATION - Greet/check in with patients at all MFM WUS
Genetics visits; serve as navigator and patient advocate to ensure
that patients attend all MFM, Fetal Testing, Genetics, Outreach,
and Special Delivery clinics; facilitate and transport patients to
various consult appointments (i.e. direct or facilitate
transportation to other clinics)
* ADMINISTRATION - Attend and be available during Special Delivery
/ MFM clinics: accompany patient from visit to visit when necessary
to ensure that she sees all the providers and clinics (particularly
important for out of town patients); coordinate visits between
outreach and UNM clinics; monitor and direct traffic between WUS,
MFM, and Journeys Monday - Friday
* COORDINATION - Be available by telephone / video conference
during all outreach sessions for provider and patient
coordination
* FOLLOW-UP - Phone follow-up post discharge as needed or requested
by family: follow-up on patient no show appointments by phone or
email
* ADMINISTRATION - Antepartum /Postpartum visits for Special
Delivery moms; face to face or phone contact with the parents while
the baby is in the NICU
* LIAISON - Serve as liaison with community services as needed; be
available as contact for primary OB; serve as primary contact for
UNM providers about Special Delivery patients
* FOLLOW-UP - Responsible for contacting families at the
appropriate intervals post neonatal loss (e.g. one month, 6 months,
Birthday, expected due date, holidays, Mother's Day and Father's
Day)
* PROGRAMS - Develop perinatal grief / hospice program for patients
with lethal anomalies and ongoing pregnancies; collaborate with
Journey's program; coordinate referrals to support groups and
treatment as needed
* SPECIAL DELIVERY - Serve as the primary point of referral for
Special Delivery patients for UNM MFM, external MFM, and primary OB
providers; create Special Delivery Summary in mother's medical
record upon entry into Special Delivery program; update Special
Delivery Summary when plans change; add Special Delivery
indications to Cerner problem list; update Special Delivery
database twice weekly; send weekly email to Special Delivery
distribution list as scheduled appointments for coming week,
updates list, etc.; ensure familiarity with each patient and
awareness of any updates or changes
* ADMINISTRATION - Send Thank You letters to referring physicians
for referring patients
* MEETINGS - Attend weekly MFM-Neonatology "In and Out" conference;
lead weekly Special Delivery Conference; attend Peds Cardiology and
NICU conferences
* SPECIAL DELIVERY FUND - Manage Special Delivery Fund. Solicit
donations to Special Delivery Fund through UNM Foundation; work
with OB department accountant to purchase gift cards,
transportation and hotel vouchers; assess patient need for Special
Delivery fund resources; distribute fund gift cards; collaborate
with MFM Chief to decide distribution of funds
* PATIENT CARE - Ensure contracted vendor receives results and
reports for abnormal screening results, abnormal carrier screen
results, and abnormal results from all genetic procedures
* PATIENT CARE - Ensure all lab orders are placed properly; monitor
lab results to ensure lab results are not missed; follow-up as
needed
* CARE MANAGEMENT - Contact patients being referred for medical,
transportation, and lodging need; introduce available services;
coordinate with Care Management
* GENETIC COUNSELING - Ensure patients are aware of Genetic
Counseling appointments by following up with reminder calls; ensure
Genetic Counseling reports are delivered to all referring providers
in a timely manner: be a liaison and resource person for questions
and answers for genetic procedures and labs; develop and maintain a
Genetics Fast Fact card by keeping the card up to date with current
facts
* PATIENT EDUCATION - Develop patient education binders; assemble
and maintain patient-specific educational material in binder
* EDUCATION - Provide on-going education to clinic staff and be
clinic resource for genetics
* PATIENT SAFETY 1 - Follow patient safety-related policies,
procedures and protocols
* PATIENT SAFETY 2 - Demonstrate proactive approach to patient
safety by seeking opportunities to improve patient safety through
questioning of current policies and processes
* PATIENT SAFETY 3 - Identify and report/correct environmental
conditions and/or situations that may put a patient at undue
risk
* PATIENT SAFETY 4 - Report potential or actual patient safety
concerns, medical errors and/or near misses in a timely manner
* PATIENT SAFETY 5 - Encourage patients to actively participate in
their own care by asking questions and reporting treatment or
situations that they don't understand or may "not seem right"

* BSN w/in 36 Months of Position

2 years directly related experience
Nonessential:
Bilingual English, Spanish, Keres, Tewa, Tiwa, Towa, Zuni, or
Navajo
5 years directly related experience
Credentials:
Essential:
* RN in NM or as allowed by reciprocal agreement by NM
* CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days
Physical Conditions:
Light Work: Exerting up to 20 pounds of force occasionally, and/or
up to 10 pounds of force frequently, and/or a negligible amount of
force constantly (Constantly: activity or condition exists 2/3 or
more of the time) to move objects. Physical demand requirements are
in excess of those for Sedentary Work. May require walking or
standing to a significant degree or requires sitting most of the
time but entails pushing and/or pulling of arm or leg controls;
and/or may require working at a production rate pace entailing the
constant pushing and/or pulling of materials even though the weight
of materials is negligible.
