*Clinical Magnet (part of ICON Medical)* is seeking a skilled and compassionate *Registered Nurse (RN)* to join an In-Home Services program in *Albuquerque, NM*.This is a *DIRECT HIRE*, full-time opportunity with a MondayFriday day schedule.Responsibilities of the Home Care RN:* Provide in-home skilled nursing care in accordance with InnovAge policies and applicable regulations* Triage participant and staff phone calls to determine care priorities* Assist with participant exams, testing, and treatments* Complete certification/recertification orders and discharge summaries* Implement physician orders and instruct patients/families on medication and treatments* Schedule and coordinate specialist appointments and hospital admissions* Perform general nursing duties including medication administration and monitoring compliance* Maintain thorough documentation and charting for all care provided* Conduct client care assessments, reassessments, and develop individualized care plans* Supervise LPNs and CNAs in accordance with regulatory requirements* Participate in care conferences, peer reviews, quality management, and utilization reviewsQualifications of the Home Care RN:* Associates degree in nursing (Bachelors preferred)* Current Registered Nurse (RN) license in New Mexico* Current First Aid and BLS certification (American Heart Association or Red Cross)* Valid drivers license, personal transportation, and auto insurance* Minimum of 1 year of experience working with the frail or elderly* 1+ year of home care experience required (No OASIS required)Compensation and Benefits for the Home Care RN:* *$45$50.48 per hour*, based on experience* *Monday to Friday, 8:00 AM4:30 PM** Full-time, direct hire opportunity* Full comprehensive benefits packageJob Types: Full-time, PermanentPay: $45.00 - $50.00 per hourExpected hours: 40 per weekBenefits:* 401(k)* Dental insurance* Health insurance* Paid time off* Relocation assistance* Tuition reimbursement* Vision insuranceMedical Specialty:* Home HealthAbility to Commute:* Albuquerque, NM 87102 (Required)Work Location: In personby Jobble
