Job Description:
We are seeking a motivated Physical Therapist to join our
healthcare team.
Provide assessment and direct patient care for individuals
requiring rehabilitation services.
Develop and implement personalized treatment plans based on patient
capacity and physician guidance.
Set achievable goals, document patient progress, and educate
patients and families about care plans.
Qualifications include graduation from an accredited Physical
Therapy program, valid MN license, and experience in long-term or
geriatric care preferred.
Proficient in EMR documentation, collaborating with
interdisciplinary teams, and delivering patient-centered care.
Must adhere to scheduled hours, maintain professionalism, and
comply with confidentiality and HIPAA.
