Job Description:
Join our compassionate team as a Hospice Visit Nurse, working
either 5p-1a or 3p-11p shifts. We foster a culture of excellence,
trust, and respect for your unique skills, offering flexible
scheduling for personalized patient care.
Assess patient needs, symptoms, and progress through comprehensive
evaluations.
Implement prescribed treatments, medications, and care plans.
Collaborate with interdisciplinary teams and community
resources.
Perform patient admissions and about five nursing visits daily.
Accurately document patient observations, responses, and
communicate with team members clearly and timely.
Maintain current R.N. licensure in Indiana and CPR
certification.
Preferred candidates have prior medical/surgical or hospice
experience.
