are looking for a Care Partner to join our Quality & Patient Safety
team in Albuquerque, NM.This job is located with Lovelace Medical
Group. A multispecialty provider group, which includes clinics,
doctors and advanced practice providers in a broad range of
specialties and primary care, and provides patients with
exceptional health care through the support of Lovelace Health
System hospitals and technology. ResponsibilitiesCare Partner Job
Responsibilities:
- Telephonic Outreach to patients for the purpose of scheduling
Medicare Annual Wellness Visits, primary care and preventative care
appointments.
- May also include chronic disease education, goal setting and
health assessment outreach calls to assist patients in changing
health behaviors and reduce complications.
- Use standing orders to assist in closing preventive care
measures, such as colon cancer screening, breast cancer screening
and routine labs.
- Assist patients in coordinating care and resources to ensure
optimal outcomes QualificationsCare Partner Education and
Experience Requirements:
- Minimum:-- High School Diploma or GED Equivalent.-- Completion
of a Medical Assistant or Practical Nursing program from an
accredited institution,
- Experience in clinic or hospital setting and knowledge of
disease processes
- Medical Equipment:-- AED, autoclave, BP cuffs (adult, infant,
and pediatric), colposcopy, cryo unit, disposable equipment
(gloves, goggles, gowns, etc), EKG machine, glycosometer,
nebulizer, nitrogen cylinder, O2 cylinder, pulmonary function
machine, pulse oximeter, scale (adult and infant), spirometer,
stethoscope, thermometer, wheelchair, and woods lamp.--
- Office Equipment:-- Computer and printer, fax machine,
telephone system, time clock, and copy machine.--
- Software:-- MISYS, E-Prescribe, and PMS
- Experience with EPIC or Electronic Medical Record
preferred.
- Computer skills including managing databases, Excel, Word.
- Excellent written and verbal communication skills.
- Customer service and telephonic outreach.--
- Knowledge of disease processes and preventive measures
preferred
- Must be a registered or nationally certified Medical Assistant
or Licensed Practical Nurse.--
- Credentials held by Medical Assistant or Licensed Practical
Nurse must require a re-certification process on a regular basis.

- Licensed Practical Nurses must hold current license and be able
to work in the state of New Mexico
