    permanent address outside of facility radius rule of 75 miles. Please note time off requests must be disclosed with initial submission to be considered for approval. Time off requested after the acceptance date will not be approved.----16
    weeks assignment, 2 years' experience current required in specialty. Must be a
    team player, flexible and accustomed to fast paced environment.-- Must have successfully completed at least one
    13-week travel assignment.Description: In
    collaboration with the Director of Case Management, provides leadership for the
    care coordination department and oversees the departments daily activities.
    Coordinates the integration of utilization management and discharge/transition
    planning functions to provide effective outcome-based patient centered care.
    Serves as the central resource to the Director of Care Management to develop
    tools, processes and systems to optimize the department and organizational
    strategic goals. --Key Accountabilities: *Ensures departmental presence and participation at daily
    interdisciplinary and case management rounds. Monitors the effectiveness of
    care coordination, utilization management and uses process improvement methods
    (PDSA) to create safe, effective and efficient processes and ensure
    standardization across areas of responsibility. Seeks continuous improvement of
    the department awareness of bed capacity issues. *Manages the daily operations
    of the department by maintaining appropriate staffing levels and assignments to
    meet the needs of the department and hospital. Refers to the current census
    report at the beginning of the shift and makes staffing changes if warranted.
    Monitors patient volume/census and case complexity/acuity fluctuations, reassigning
    staff as appropriate to ensure cost effective resource utilization. *Directly
    manages department staff and selects orients, trains, mentors, supervises, and
    assesses competencies *Participates in the Hire Right Process as established by
    the organization and assists with education, orientation, onboarding and
    ongoing development of new staff *Demonstrates a professional approach,
    promotes a customer-service environment and maintains an open working
    relationship with internal and external customers, physicians, and staff
    utilizing appropriate resources in the chain of command for problem solving.
    *Maintains professional growth and development through seminars, continuing
    education classes, conferences, and professional journals. *Initiates employee
    progressive discipline and counseling procedures as needed and participates in
    all phases of personnel management. *In collaboration with Director,
    establishes work procedures and writes departmental policies and guidelines.
    Monitors processes for compliance with CMS or other oversight agencies.
    *Performs other functions as required. Response

    lifestyle, socioeconomic, age, and value differences in practice. CERTS: Bachelor's Degree in
    nursing required; Master s preferred. Must hold current License from State of
    New Mexico. Five years of relevant acute care clinical experience required.
    Relevant management/leadership experience required. Three years of Case
    Management or Utilization Management experience, in an acute hospital or
    relevant setting.SHIFTS: 8-10-Hour Day Shift; Shift
    start: 8am Duration: 8-10hrsWeekends: Yes, rotation basis. --Holidays: Yes Call: NoShifts may fluctuate to fulfill
    department operational support and patient needs. Weekends and holidays may be
    required and time off to be coordinated with other leaders to ensure coverage
    of department is guaranteed.--

    ORIENTATION:-- First week of the contract, not to exceed 40
