DescriptionThe Telephonic Social Worker, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Telephonic Social Worker handles assignments which are varied and frequently require interpretation and independent determination of the appropriate courses of action.ResponsibilitiesThe Telephonic Social Worker employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. Will create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.Required Qualifications

  • Master's degree in Social Work .
  • Current valid social work license, e.g. MSW, LCSW, LMSW
  • Minimum 2 years of experience in identifying and accessing resources to meet basic needs and in securing community-based assistance.
  • Proficiency with Microsoft Office Programs, e.g. Word, Excel, PowerPoint, Access
  • Schedule: Monday-Friday, 8 hour shifts varying between 9AM and 8PM EST. Based on business needs, occasional weekends may be required.
  • Additional licensing requiredWork-At-Home Requirements
    • WAH requirements: Must have the ability to provide a high speed DSL or cable modem for a home office. Associates or contractors who live and work from home in the state of California will be provided payment for their internet expense.
    • A minimum standard speed for optimal performance of 25x10 (25mpbs download x 10mpbs upload) is required.
    • Satellite and Wireless Internet service is NOT allowed for this role.
    • A dedicated space lacking ongoing interruptions to protect member PHI / HIPAA informationPreferred Qualifications
      • 3-5 years of experience working as a licensed Social Worker
      • Experience with behavioral change, health promotion, coaching and wellness
      • Experience working with adults, and aging population preferred
      • Knowledge of community health and social service agencies and additional community resources
      • Bilingual English/Spanish. Must be able to speak, read and write in both languages without limitations or assistance.
